As the Dwyane Wade farewell tour made its final stop in the city of Portland, Trail Blazers superstar Damian Lillard was the honorary recipient of the traditional post-game jersey swap from the soon-to-be-retired Wade. Following the Miami Heat's 118-108 victory against the Blazers, it was Lillard who was given the honor of having to bring home one of Wade's last few game-worn jerseys.

Wade had one of his better games of the calendar year, with the 37-year-old posting 22 points on 9-of-17 shooting. The 6-foot-4 guard also collected nine rebounds, three assists, and a block in 29 minutes off the Miami bench. The three-time NBA champion made sure to make his last trip to Portland a memorable one.

On top of Wade's vintage performance, the Heat were also led by big man Hassan Whiteside's huge 28-point, 11-rebound double-double. The 7-footer was also active on the defensive end and was able to log four steals and two blocks to round out another impressive performance.

For his part, Lillard had a terrible outing against Wade and company. The All-Star point guard managed only 13 points on 5-of-15 from the field and just 2-of-10 from distance. He did record three rebounds and 10 assists, but also turned the ball over four times.

It's going to be a busy next few days for Wade and the Heat, as they are slated to take the floor three times in four days starting with the Sacramento Kings on Friday. With Wade's maintenance plan in full swing, it would not be surprising if he ends up sitting out at least one of his team's next three games for rest purposes.
